Athletic Registration via FinalForms

Physical Form

We are excited to partner with FinalForms, the district's online athletic registration platform. Please visit https://laconner-wa.finalforms.com.

Step by Step Instructions:  PARENT PLAYBOOK

  1. Go to https://laconner-wa.finalforms.com  (First time? Click *NEW ACCOUNT). 

  2. Follow the registration steps, including enabling translation if English is not your first language.

  3. You will receive an email from FinalForms within about 5 minutes prompting you to confirm your account. If you do not receive an email, check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.

  4. Set up your new FinalForms password and select Confirm Account.

  5. Select Register Student, then type your student's basic contact information and select Create Student. 

  6. Check the box next to any sport(s) your student plans to participate in and select Update. Please note: a selection can be changed any time until the registration deadline; after that please contact the Athletic Secretary to add a sport.

  7. Your student will now need to login to their email and follow the instructions in their confirmation email to create their password. * Important * An email will automatically be sent to the email address that you provided for your student, which will prompt your student to sign their portion of the athletic forms.

  8. Students and parents/guardians will need to complete each form and sign their full name in the signature field at the bottom. After signing, select Submit Form and move on to the next form. 

Note: A GREEN check will appear when the form has been signed by both guardian and student. Forms awaiting student signature will appear in YELLOW.  Forms in RED still need to be signed by parent/guardian AND student. 

Once all available forms are complete, you will see a Forms Finished message. This is not necessarily a confirmation of clearance or eligibility through the Athletic Department.

  • Athletes will be required to pay an athletic participation fee. The fee must be paid prior to the day of the first competition. Students will also be required to purchase an ASB card. We recommend purchasing the ASB card at the beginning of the school year to take full advantage of its benefits. Students with an ASB card can get into events at the high school free or at a discounted rate.

  • Please turn in a completed physical form from your physician. Physicals are good for two years. Call the MS/HS Office or email us at mshsoffice@lc.k12.wa.us to verify if your physical is still valid. Physicals must state that the student is cleared to participate in sports, and be turned in BEFORE the first day of practice.
